Issue of Disability Certificates:The facility of certification for disability started on 1/3/2010 Disability board was formed comprising of specialist of various disciplines.

At present, the areas covered are

• Rithala Assembly
• Rohini Assembly
• Pooth kalan,
• Begumpur, (part of Bawana assembly)
• All sectors of Rohini (Sector 1 to Sector 25)

Disability certificates are issued in respect of disabilities related to following departments. Name of the doctors designated to issue disability certificate are also mentioned.

• The person desirous of getting Disability Certificate may enquire from dealing assistant present in Room No. 2156 for feasibility of getting a certificate as per his/her residential address.
• If yes then the person has to fill an application form and submit the form with all necessary documents including two passport size photographs and proof of residence.
• The form is submitted in Room No. 2156. At the time of submission of form the person is informed about the date and time to appear before the medical board.
• The OPD card is made on the schedule date and is examined by the medical board.
• For orthopedic cases in the first visit apart from physical examination X-Rays and Physiotherapy evaluation is also done. Within a week X-Ray and physiotherapy evaluation reports are collected by the department and the applicant has to physical present in front of the disability board with the reports. Normally it takes another week for issue of disability certificate to the applicant.
• For issue of disability certificate related to Eye/ENT the applicant is given the date to show in the respective OPD at earliest. When the applicant submits application form an OPD card is made on same day and applicant is examined by concerned specialist and tests are done as per requirement Disability certificate are issued within a week.
• For pediatric/mental retardation cases, where ENT evaluation is required, the cases are referred to Guru Gobind Singh Hospital/G B Pant Hospital/LNJP Hospital for BERA (Brain stem Evoked Response eudiometry).
• For Neurological and psychiatric disability, patients are referred to IHBAS, Dilshad Garden. The patient has to report at IHBAS OPD Room No-21-A or 30 for disability assessment. The assessment process takes around 2-3 weeks subject to regular follow up of the applicant at IHBAS OPD. The cognitive impairment is categorized as mild, moderate, severe & profound. The exact percentage of disability is to be decided by the board members at Dr BSA Hospital.
• For Burns and Plastic Surgery &multiple disabilities including neurological, patients are referred to LNJP hospital.
• In case patients withdisability in eye require some tests which are not available in the hospital but are necessary to assess disability status of the patient such patients are referred to Guru Nanak Eye Centre.

Application for obtaining disability certificate is to be submitted as per Form'VI',part IV,Delhi Gazette.

• In cases blindness and amputation of limb or complete permanent paralysis of limbs disability certificate are issued in form VII.
• For multiple disabilities, certificates are issued in form VIII.
• In cases of disabilities other than which is mentioned in form VII or VIII, certificates are issued in form IX.
• Intimation of rejection of application for disability certificate is done on form X.

MACT (Motor Accidental Cases Tribunal)cases are also received from Rohini court for issue of disability certificates. InMACT,cases from all parts of Delhi are accepted and disability certificates are issued as per the guidelines. In case where patient is residing out-side the Delhi but accident occurs in Delhi, such cases may be referred by MACT court to our hospital. (Till now 707 disability certificates have been issued by our hospital to MACT cases).
